You need to complete, sign and return all three pages of our New Customer Package along with a copy of your driver’s license and the credit card listed on page 1. A downloadable Adobe pdf e-Form is available by clicking the grey “Application” box in the tabs bar at the top of this page.

Be sure to include the Tax Exemption Form. This is necessary because anything in the State of Texas used for film, video, or motion picture production is automatically exempt from sales tax by law. However, we are required by the State to keep an Exemption Form on file for every client who receives this benefit.
Our verification process for new customers typically takes less than an hour on weekdays during normal business hours. The process can take longer on Saturday, after-hours, or high-volume days. Once your account is set-up, we’ll send you a confirmation email.
We accept Visa, MasterCard, American Express & Discover, as well as debit cards, cash, and company checks. We do NOT accept personal checks, prepaid, reloadable, or virtual cards.
Yes. The person paying for the rental must complete, sign and return a Credit Card Authorization Form along with a photo of their driver’s license, plus the front and back of the credit card listed on the form.


Reservation can be made as far in advance as you’d like. The further in advance you reserve items helps ensure everything will be available on the dates you need them.

You will be able to select the pick-up and return dates at checkout when submitting a rental inquiry through our website.
Yes. However, we strongly suggest you submit a rental inquiry online or talk to one of our Rental Coordinators by phone (214-654-9977) to check availability before coming to our facility. This will ensure everything is available and we can have it ready when you come in.
Reservations may be cancelled at any time up to 24 hours prior to pick-up, or delivery at no charge.

Cancellations 24 hours, or less from the scheduled pick-up, or delivery date/time are subject to a 50% cancellation fee of the first day’s rental charges.

Orders cancelled after pick-up, or delivery are subject to a half-day (.66 the day rate) for 1-day rentals and 1-day minimum charge for multi-day rentals.
1-day rental – Pick-up after 3 pm the day before your shoot. Return by 10 am the day after your shoot.

2 to 7-day rental – Pick-up after 3 pm the day before your shoot. Return by 10 am the day after your shoot. (Example: If your 7-day shoot starts on Wednesday you can pick-up the equipment after 3 pm on Tuesday and return it by 10 am the following Wednesday).

Weekend rental – Pick-up after 3 pm on Friday. Return by 10 am Monday.
A deposit is required for all new customers until a relationship is established. The amount of the deposit is based on a percentage of the replacement value of the equipment rented.

Deposits are NOT required for established clients.
No, international rentals are prohibited. MLD equipment is only available for rental inside the US and its territories.
MLD does NOT offer damage/loss insurance. Your financial responsibility is limited to a maximum of $5,000 for equipment damage and/or loss per each rental contract (subject to the exclusions below). For the items listed below you are fully responsible for 100% of the cost of replacement or repair:
– lens scratches
– unauthorized internal adjustments, or repairs to equipment
– willful, or intentional misuse, or loss
– unexplained disappearance
– fraudulent, or dishonest acts
– warlike, or government action, such as confiscation, or seizure
– theft which is unreported to the police within 48-hours (valid police report required)
– theft from any closed vehicle (such as a truck or van) is not covered unless the vehicle
is locked & there are signs of forcible entry (valid police report required)
– theft from a visually open vehicle (such as an SUV, van, or sedan) is not covered under
any circumstances
– up to $5,000 regardless of damage, or loss for any reason while equipment is in the
hands of a common carrier

Equipment returned to MLD with a Shockwatch label activated or removed is subject to an Engineering Inspection Fee (up to $200) to determine if it has been damaged. If damage is found, you are liable for repairs under the terms listed above. Removed or triggered Shockwatches are subject to a $25 replacement fee.
While insurance is not required to rent from MLD, if you have a Certificate of Insurance (CoI) that covers rented equipment we will discount the rented items total by 10%. Personal insurance policies such as renters or auto insurance cannot be used.

To meet our requirements the certificate must list the following:
– At least $25,000 in coverage for “rented equipment in-transit and various locations”
with MLD named as Loss Payee
– A minimum of $1,000,000 in general liability coverage with MLD named as Additional

Yes, qualified students can receive 20% off all rental items. To qualify we require a letter from the professor or teacher on school letterhead stating the rental is a class project for course credit.

Pick-ups & Returns

Equipment can be picked up and/or returned in person from our facility during normal business hours (i.e. 7:30 am to 6 pm weekdays, or 9:30 am to 1:30 pm on Saturday).

Orders over $300 can be dropped-off or picked-up within 50 miles of our facility for free. For orders under $300 we charge $35 per trip. Delivery/pick-up service is only available weekdays between the hours of 10 am and 5 pm.

If you cannot pick-up or drop off during our normal business hours, after-hours service is available at our facility for a $200 fee. Delivery and pick-up services are not available after-hours.
If someone else will be picking up your rental, please let us know as soon as possible and provide the name of the individual. Please note, in some cases the cardholder needs to be present at pick-up.
Yes. Our preferred methods are SW Airlines Cargo, FedEx, or UPS. We do not ship equipment via the United States Postal Service, or to P.O. Boxes. Delivery signatures by an adult are required for all shipments.

If you need us to ship equipment, we will be happy to provide a quote for the level of service you request.
In most cases, you can extend your rental unless the equipment has been booked by another client for the extra days you request.

Rental Support

Each piece of MLD equipment has been thoroughly tested to make sure there are no issues before it goes out on a rental. However, in the unlikely event an item is not working as expected call or email us immediately for assistance.

If an issue arises outside our normal business hours (i.e. 7:30 am to 6 pm weekdays, 9:30 am to 1:30 pm on Saturday), a member of our staff is on-call 24/7/365. Simply dial our main number 214-654-9977. When you get to the afterhours voicemail prompt press 2 to go to the emergency mailbox and leave a message with your name, phone number and a description of the problem. The on-call person will be notified and promptly get back to you.
We will be happy to provide an orientation at our facility for the set-up and operation of any piece of equipment we carry. Simply let the Rental Coordinator know you would like an orientation at pick-up.
